Here is a link to my TpT account where you can download my FREE reader's workshop conference log and cover page! I generally print out 2 sheets per student at the beginning of the year, punch holes in them, and keep them together in a binder. (For those teachers who are EXTRA organized, I divide the student logs up using dividers based on their guided reading groups!)
